1. Representation & Conduct Expectations
By attending this event, I understand that I am representing Smile Brands or its affiliated Dental Practices (collectively “Company” or “Smile Brands”) and agree to conduct myself in a manner that upholds the Company’s reputation, values, policies, and Code of Conduct. I acknowledge that this is a Company-sponsored function, not a vacation, and I will act professionally throughout, including during social or off-hours portions of the agenda.
2. Alcohol Use, Safety & Responsible Behavior
I acknowledge that alcoholic beverages may be made available during the event. I agree to:
- Consume alcohol responsibly and in moderation.
- Maintain sound judgment and ensure the safety of myself and others.
- Avoid any behavior that endangers myself or others.
- Refrain from illegal conduct (e.g., driving under the influence).
- Seek assistance from event leadership, staff, or security if I—or another person—becomes impaired.
I understand that alcohol consumption is optional, and I remain personally accountable for all my decisions and actions while participating in the event.
3. Release & Waiver of Liability; Indemnification
To the fullest extent permitted by applicable law, I hereby release, waive, and forever discharge Smile Brands, along with its officers, directors, employees, agents, affiliates, and assigns (collectively, the “Released Parties”), from any and all liability, claims, demands, actions, or causes of action (known or unknown) arising from or relating to my attendance at or participation in this event—whether arising from alcohol consumption or the conduct of others.
I further agree to indemnify, defend, and hold harmless the Released Parties from and against any losses, damages, liabilities, costs, or expenses (including attorneys’ fees) stemming from my misconduct, intoxication, negligence, or breach of this Agreement, whether caused by me or others.
This release covers claims even if a Released Party is alleged to have been negligent, except where prohibited by law. This release does not apply to claims for workers’ compensation or other non-waivable statutory rights.
